Hearing a reference to an injury brings back Amuro’s memories of visiting the clinic. The flashback reveals that he deliberately got hurt to see Elena. Elena is pregnant with Shiho at the time, while she and Atsushi discuss moving to a research facility.
The unidentified doctor from Volume 84 is now connected to three distinct roles: Ai Haibara’s mother, a future Black Organization scientist, and Rei’s first love. These roles converge in one quiet memory of the clinic.
CASE: Rei Furuya Explains One Reason He Wanted to Be a Police Officer
The police academy story reveals that Rei’s decision to become a police officer was connected to a woman who had suddenly disappeared. He tells Jinpei Matsuda that he is looking for her. Although he does not name Elena in that conversation, the clinic memories make her the strongly implied person.
From the child’s perspective, Elena simply went far away without explaining her destination. Police work offered a path toward finding the truth. The manga strongly implies that searching for Elena was one starting point for his police career; it does not establish that she explains every later choice.

Bourbon knows that Elena was called Hell Angel within the Black Organization. During the Mystery Train confrontation, he addresses the apparent Shiho as Hell Angel’s daughter. That wording shows knowledge of Elena’s alias and her parent-child relationship with Sherry.
For Rei, the kind doctor at the Miyano Clinic and the scientist known as Hell Angel had already become the same person. The story does not show when or through which assignment he learned this.

This encounter cannot establish any exchange of secrets between Amuro and Haibara. Amuro thought he was addressing Shiho, but the actual listener was Kaitou Kid.
Confirmed Childhood Acquaintances Are Elena, Atsushi, and Akemi
The Miyano family members Rei is confirmed to have met are Elena, Atsushi, and Akemi. Bourbon also tells the apparent Shiho that he has met her parents and sister.

Combined with his introduction immediately beforehand, the statement distinguishes those three acquaintances from Shiho. It would therefore be inaccurate to say that Amuro had always known every member of the Miyano family.

There Is No Confirmation That Amuro Knows Haibara Is Shiho
No scene confirms that Tooru Amuro identifies Ai Haibara as Shiho Miyano. On the Mystery Train, Bourbon was led to believe Sherry died in the explosion, and no later scene shows him learning that she survived.
In Volumes 101-102, “Takagi and Date and the Notebook Promise,” Amuro and Haibara occupy the same space at Café Poirot. The anime adaptation is Episodes 1167-1168 (Japanese episodes 1109-1110), “Takagi and Date and the Notebook Promise (Part 1)” and “Takagi and Date and the Notebook Promise (Part 2).”
His look can seem suggestive, but he never states that he recognizes her as Shiho. The story does not resolve whether he notices a resemblance to Elena or is watching her for another reason. Suggestive attention is not the same as confirmed knowledge of her identity.
The movie Black Iron Submarine raises suspicion within the organization about Haibara and Shiho being the same person, but the evidence discussed here does not give Amuro a clear moment of recognition there.
